Counting

What is the issue with counting the contents of shipments? Why is not only a monumental task to get a shipment counted properly, but also takes a miracle just to get something corrected after it's counted wrong? Shipments seem to be counted wrong all the time, so why is the process to correct something so pervasive just as problematic? So much so that Amazon will just default to telling the sellers that they are wrong without any REAL further examination?

Any amount of insight would be helpful.
