Buyer Fraud, Buyer returned 100% of 40 orders over 6 months!!!!! 43 items returned and 95% of the items were swapped or heavily used.
I have reported the Buyer Every time but nothing happens, buyer continues to abuse me and Amazon is complacent.
What are my options? Who do I need to escalate this issue to?

I don't have a solution for Amazon allowing theft and fraud BUT if this was shipped using the USPS please report the issue to the Postal Inspectors as mail fraud. If not, report it to the carrier.
ALSO the Government has a site IC3 (Amazon will remove the link if I put it here but it’s part of the FBI for an easy search) where you can report the use of the internet for theft if it was UPS, FedEx or whatever. It is generally used to report on websites but it can be used to report buyers as well if you happen to encounter one of the Amazon scam crowd.
In all cases it at least puts the con artists into a data base for future reference by the authorities.

Can't you file a Safe-t claim?
If it's clear as day it's been tampered with you'll win. We win 92% of all safe-t claims.
If this is gray area start taking photos of the parts being shipped out.
If they weren't RFS then give them .01 back and send a message explaining why. Take photos and send them so when / if an A2Z claim comes in you will have photographic evidence. If you continue to "allow" it they will continue to abuse it.

I know you cant share the results of your investigation, but from the outside looking in when a buyer returns 100% of your orders and returns completely different items, this buyer should have been banned months ago.
Stop shipping to them, cancel their orders. Hopefully you have enough volume that a few cancels will not hurt you.
I'm getting it every single day. I have 14 open returns right now. I've been selling online full time over 25 years. This is unprecedented. Until relatively recently, we could go months without a single return.
I'm going through today's returns now, and three of the four have swapped parts and returned different items.
Sometimes, it's a used version of what we sold; sometimes, it's random stuff in a box for weight; other times, it's just people who used us as a computer rental service.
It's been getting worse for the past few years. This past two weeks, a large percentage of our sales have been from people or groups using us to drop-ship items to their customers. The drop shipper then opens a return and sends back broken, damaged, or different items for a refund.
For the life of me, I can't think of any way to stop it.
Hire a lawyer and go after the buyer - it's a slam dunk case. Go after them for cost of the product, all your attorney fees, and time.
