Not fair for booksellers Pre-fullfillment cancel rate
Having used Biblio, Abebooks, Alibris and of course ebay, they all have a more lenient Pre-fullfillment cancel rate, or in lay words, not being able to send out a media item because it is not in your inventory. This is a bit crazy that Amazon is so strict on this for FBM sellers. Not fair, um yes please. I'm upset about this because it has put my account at risk. I will work harder to try and fulfill all orders, but these other sites are giving you more chances than Amazon, and [Moderator Edit: removed inappropriate commentary]. Ebay, takes more of your money, but lets it slide. This is the only part of Amazon I don't like, I love everything else. This attempt to force you to go to FBA, is uncalled for in my opinion.

One thing I do to avoid trouble -- as soon as I get an order from any site is to instantly delete the listing from other sites.

Yes, it's annoying when you have to cancel an order because you don't have the book. It happens to all of us; sometimes because it sold elsewhere, sometimes because you messed up in listing it, sometimes because the book is no longer in sellable condition; but if it happens so often that your Amazon account is in trouble, then the problem is YOU, not Amazon.
I've been here for nearly a decade; had to cancel maybe a dozen times because I didn't have the book; but in fairness, I'm pretty small. But if you are big, a few cancels/week won't put your account in danger; if you're small enough that 2 in 14 days is a repeating issue, then you really should be able to control your inventory better.

So to be having a problem, that means that you are losing 5+ books in a month! Not good.
Which I stated above. But if you're losing 5-10 books in a month, you really need to check your inventory. Expecting Amazon to not care if you cancel 1 out of every 40 orders is a bit absurd.

Let's all sink to the lowest common denominator. The others are "pushovers' so Amazon should be one as well.
Amazon is ridiculous in a LOT of ways, but for someone to say that they should sink to the lowest of the low, just to allow sellers to get away with lacking adequate inventory controls is absurd.
Find a software company that can integrate your inventory across multiple sites and pay whatever they want from you for their services. I'm paying about $300/month for Ordoro and they integrate with Amazon, eBay, and, another site I sell on. I have one other site that I have to manually adjust because it isn't linked in their system yet.
I rarely have OOS and when I do it is usually because I mistyped a SKU and the computer didn't find the item as a result.

You are right. It stinks when you can’t find a book in inventory or the book has a blemish that changes its condition that you didn’t notice when you first listed it. It stinks. But if we want to sell here, we have to play by the rules. It used to be that we could drop ship from Amazon itself, another seller, or another site. But they have since made that against the rules. So, if we can’t ship, we have to take the hit. It’s that simple. Amazon will not change any of its policy or procedures to make selling easier for us. It’s just the way it is. Again, it stinks, but it’s what we got.
If you can’t deal with their rules, stick with the other sites.

They are not forcing you to go to FBA, they are forcing you to have stock in hand when selling something. Evidently you are selling it elsewhere and not deleting it from Amazon in a timely fashion. There are programs out there that can do this for you (for a fee).
