Letter of Guarantee from manufacturer - product approval

I recently had one of my dietary supplements taken down by Amazon to get approved like many sellers have been experiencing lately. I got a COA from an ISO certified lab & product images won’t be an issue but I am having trouble with the Letter of Guarantee from manufacturer(LoG) This is what Amazon is asking for:
Letter of Guarantee - A letter of guarantee from the brand owner of the product that meets the following requirements:

  1. Is on official brand owner letterhead.
  2. Contains complete product name as listed on label, or list of products manufactured under the brand or by facility.
  3. Assurance that the product is manufactured under current Good Manufacturing Practices as listed in 21 CFR Part 111.
  4. Assurance that only lawful and safe, as defined in section 402 (f) of the FD&C Act , ingredients are utilized.
  5. Assurance that concentration of active ingredient(s) as stated on the label and labeling is safe for consumption.

Would it be possible for someone to post an example of your LoG that has been approved by Amazon with any important information blacked out? My manufacturer has never made a LoG before. I’ve seen a lot of examples online but I don’t know if they meet amazons requirements/ requirements for dietary supplements. My manufacturer has no problem writing out the document for me but would like to see an example so that the can create it following amazon standards. Any help would be great on this matter.

Is this a private label brand? If so, YOU are the one writing the letter as the brand owner on your letterhead. Below is what we did for our PL brands and it worked. Can easily be adapted for a 3P brand. Your 5 points above are interesting to me. Apparently Amazon has changed them from what they were just 2 weeks ago… Go figure. Point 1 was (ON YOUR LETTERHEAD (Seller). Point 2 didn’t have "or list of products manufactured under the brand or by facility.

We added more than what was requested (area in bold). It’s not always a good idea to give more than what’s asked for but we took a chance since our supplier is NSF / GMP certified.
