While the metric being above 1% will be considered off target, it will not mean that action is immediately required from you. I encourage you to review the metric and download the report to see what orders are specifically impacting you. The metric will be calculated off of three separate areas: negative feedback, A-to-z guarantee claims, and chargeback claims. If you identify any specific issues or trends that are contributing towards the impacts, I would advise on taking action to correct those concerns.
An appeal won't be required unless it has been explicitly requested from you. Generally, there are warning banners and other type messaging that will ensure you are aware of the potential risk to the account. Are you seeing any banners on the account health page? Have you received any requests for information through your performance notifications?
