UK VAT Reclamation- Please Help

I am a new US based seller, selling into Amazon UK. I have a EORI and VAT. I am using a freight forwarder, who is using FedEx to get my goods into UK.

How do I know that my property EURI are being used on the VAT and duty? Speaking with my accountant, they are telling me I need a c79 form that will show if indeed my EURI is being used but I am not clear where to get this form? I want to obviously prevent a problem before one begins.

Please advise.

Respectfully I can google c79 form as well. My question is, how do I know when I send a shipment and then pay a VAT bill, that my proper EURI was used?

It seems the only customs doc accompanying my shipment is a commercial invoice, I have never received additional customs docs. So if my EURI appears on the Commercial Invoice is that sufficient?

Not sorry, you obviously have issues reading or using the Google. Where do you get the form from? One of the first or second links tell you that.
