Can someone point me to the requirements of labeling the bottle of my product. For example, does my bottle have to have an address on it?

I’m unaware of any Amazon specific product labeling requirements (outside of the FBA bar code sticker). You need to Google "The Fair Packaging and Labeling Act (FPLA or Act). It was enacted in 1967 and directs the Federal Trade Commission and the Food and Drug Administration to issue regulations requiring that all “consumer commodities” be labeled to disclose net contents, identity of commodity, and name and place of business… You might also look-up “U.S. FDA Labeling Requirements” or “Guidance for Industry: Food Labeling Guide”. Since we don’t know what’s in your bottle, I’m not sure that we can help.
I would not do product labeling alone without help from a consultant or labeling expert. There are loads of requirements for most products. You need to make sure it is done correctly. Don’t rely on an offshore manufacturer (or a US based maker) to get the product label correct. The responsibility to meet federal and state regulations and guidelines is yours alone.

Labeling is not the real problem. The problem is what’s IN your bottle. When you take the contents to the USA based testing lab, they will tell you what must be placed on the packaging free of charge.
